Toenail fungus often starts on the skin and then spreads into the nail. It is usually caused by a group of fungi called dermatophytes, which infect the outer skin layer (stratum corneum), then move into the deeper layers.
Once established, the fungus travels through the skin, attaches to nail keratin, and begins to grow. It releases enzymes that break down keratin so it can use it as food, allowing the infection to multiply and spread across the nail.
